Job summary

Continuum Therapy Partners is seeking a licensed Physical Therapist for Meadow View Nursing Center in Berlin, PA. This part-time daytime role focuses on evaluating and implementing individualized treatment plans in coordination with other disciplines.

Requirements include a Doctor of Physical Therapy (DPT), a state license (NPTE pending), and 1 year of PT experience preferred. A $5k sign-on or $10k retention bonus is offered along with standard benefits.

Qualifications

• Doctor of Physical Therapy (DPT) degree required.
• Current, unencumbered state Physical Therapy License; must pass the NPTE.
• 1 year of previous experience as a Physical Therapist preferred.

Responsibilities

• Evaluates, develops, and implements individualized treatment plans in coordination with other disciplines.
• Documents patient progress and adjusts plans of care based on clinical outcomes.
• Educates patients and families on home exercise programs and injury prevention.
