Job summary

The Pines Home Health and Hospice is seeking a licensed Physical Therapist to deliver in‑home therapy across Arizona. You will design individualized care plans, perform hands‑on therapy, and coordinate with physicians, while maintaining strong patient and family education and documentation.

We offer scheduling flexibility, autonomy in daily patient management, and a supportive team that values mentorship and growth in a home health setting.

Qualifications

• Licensed Physical Therapist in AZ.
• CAPTE-accredited program; passed state-approved exam.
• Minimum 2 years’ experience (home health preferred).
• Current CPR certification.
• Fingerprint Clearance Card (or immediate ability to apply).
• Valid driver’s license, insurance, and reliable transportation.

Responsibilities

• Develop and update individualized care plans.
• Provide in‑home physical therapy services per physician’s plan of care.
• Assess functional status, mobility, and home environment.
• Educate patients and families on therapy techniques and equipment.
• Maintain accurate documentation and communicate progress to physicians.
