Skylake Post Acute in Thornton, Colorado, is seeking a part-time or PRN Physical Therapist to evaluate residents, develop individualized treatment plans, and provide high-quality PT interventions. The role supports a large SNF with 24-hour skilled nursing care and an emphasis on rehabilitation outcomes.

Qualified candidates hold a Colorado PT license, have completed an APTA CAPTE‑accredited program, and ideally possess experience in post-acute or long-term care settings.

Formación

• Current, unencumbered Colorado Physical Therapist License.
• Graduate of an accredited Physical Therapy Program by APTA & CAPTE.
• Experience in post-acute or long-term care environment preferred.
• Ability to pass a Colorado CAPS background check.
