Position Overview

We are seeking a Full-Time Outpatient Physical Therapist at Champions Healthcare, Willowbrook, Houston, TX. Our facility offers long-term care, post-acute rehab, and geriatric outpatient therapy services. Join our stable, in-house therapy team dedicated to providing excellent care and supporting our geriatric patients' recovery journeys.

Responsibilities

1. Conduct screening and evaluation of patients with physical disorders and/or wound care needs, developing care plans that meet regulatory and clinical standards.
2. Deliver rehabilitative, skilled, and medically necessary treatments according to clinical requirements.
3. Administer procedures including manual therapy, ambulation, therapeutic exercises, modalities, supportive devices, and wound care.
4. Provide consultation and counseling to patients, families, and caregivers regarding physical conditions.
5. Prepare comprehensive discharge summaries ensuring compliance with regulations.

Qualifications

• Physical Therapy license required.
