Job summary

Confluent Health is seeking a compassionate physical therapist to join our outpatient clinic team in Jeffersontown, KY. New grads welcome; we value patient-centered care and help patients return to optimal function through evidence-based interventions.

You will work Monday through Friday with a competitive compensation range of $72,000 to $90,000 plus up to $10K sign-on bonus, with opportunities for certifications, mentorship, and continuous professional growth in a supportive environment.

Qualifications

• Licensed Physical Therapist (or ability to obtain licensure in the state of practice)

Responsibilities

• Evaluate, diagnose, and treat patients with musculoskeletal, orthopedic, neurological, and sports-related conditions.
• Develop personalized treatment plans based on assessments and patient goals.
• Implement evidence-based interventions to improve mobility, strength, and function.
• Monitor patient progress and adjust plans as needed.
• Educate patients on self-management, home exercises, and lifestyle changes.
• Collaborate with physicians, nurses, and staff for coordinated care.
• Maintain accurate documentation in compliance with clinic policies.
• Provide guidance and mentorship to support staff and ongoing professional development.

• Develop personalized treatment plans based on thorough assessments and patient goals.
• Implement evidence-based therapeutic interventions to improve mobility, strength, and overall function.
• Monitor patient progress and modify treatment plans as necessary.
• Educate patients on self‑management techniques, home exercises, and lifestyle modifications to enhance long‑term recovery.
• Collaborate with other healthcare professionals, including physicians, nurses, and support staff, to ensure coordinated and comprehensive care.
• Maintain accurate and timely patient documentation in compliance with clinic policies and regulatory requirements.
• Provide guidance and mentorship to support staff and assist with ongoing professional development

Compensation: $72,000 - $90,000 + inclusive of productivity incentives, potential sign on bonus, with additional compensation opportunities for certifications and location differentials .
