Confluent Health is seeking a compassionate Physical Therapist to join our outpatient clinic team in a growing Fairborn community. The ideal candidate will evaluate, diagnose, and treat musculoskeletal and neurological conditions with evidence-based interventions to restore mobility and quality of life.

Qualifications

• Licensed Physical Therapist in the state of practice or eligible for licensure.
• Evidence-based, patient-centered care approach.
• Outpatient clinic experience preferred.

Responsibilities

• Evaluate, diagnose, and treat patients with musculoskeletal, orthopedic, neurological, and sports-related conditions.
• Develop personalized treatment plans based on assessments and patient goals.
• Implement evidence-based therapies to improve mobility, strength, and function.
• Monitor progress and adjust plans as needed.
• Educate patients on home exercises and lifestyle modifications.
• Collaborate with physicians, nurses, and support staff for coordinated care.
• Maintain accurate documentation in compliance with policies and regulations.
• Provide mentorship and support to clinic staff.

• Evaluate, diagnose, and treat patients with a wide variety of musculoskeletal, orthopedic, neurological, and sports-related conditions.
• Develop personalized treatment plans based on thorough assessments and patient goals.
• Implement evidence-based therapeutic interventions to improve mobility, strength, and overall function.
• Monitor patient progress and modify treatment plans as necessary.
• Educate patients on self-management techniques, home exercises, and lifestyle modifications to enhance long-term recovery.
• Collaborate with other healthcare professionals, including physicians, nurses, and support staff, to ensure coordinated and comprehensive care.
• Maintain accurate and timely patient documentation in compliance with clinic policies and regulatory requirements.
• Provide guidance and mentorship to support staff and assist with ongoing professional development

Qualifications

• Licensed Physical Therapist (or ability to obtain licensure in the state of practice)
• Passion for evidence-based, patient centered care

Compensation: $78,000- $88,000 + inclusive of productivity incentives, potential sign on bonus up to 10K with additional compensation opportunities for certifications and location differentials. Benefits include generous paid time off, medical, dental, vision, disability, and 401(K) with a 4% employer match.
