Job summary

ChristianaCare Health System in Middletown, NY, is seeking an outpatient Physical Therapist to provide rehabilitation services in a blended neurologic and orthopedic clinic. Two evenings per week are required.

The role involves evaluating patients after injury or disease, creating treatment plans, documenting care, and educating patients and families while coordinating with referral sources to ensure continuity of care.

Qualifications

ChristianaCare is currently hiring a full time outpatient Physical Therapist at the Middletown Rehabilitation Site. Two evenings per week are required.

Primary Function:

To provide Physical Therapy for individuals requiring rehabilitation after injury or disease process. In this position you will be treating neurologic patients in a blended neurologic and orthopedic clinic.

Principal Duties and Responsibilities:

• Evaluate and re-evaluate patients in a timely manner
• Establish appropriate treatment programs that include goals to be attained
• Complete all documentation relating to patient care, including evaluations, progress notes, and discharge summaries.
• Review active patient records
• Provide patient and family education and training
• Maintain written and verbal communication with referral sources concerning patient care

Education and Experience Requirements:

• BS, Masters, or Doctorate degree in Physical Therapy
• Licensure or eligibility for licensure in Delaware

Hourly Pay Range: $36.10 - $57.76This pay rate/range represents ChristianaCare's good faith and reasonable estimate of compensation at the time of posting. The actual salary within this range offered to a successful candidate will depend on individual factors including without limitation skills, relevant experience, and qualifications as they relate to specific job requirements.
