We are seeking a physical therapist to join M Health Fairview Rehabilitation Services in Golden Valley and Eden Prairie! Golden Valley is an energetic multidisciplinary rehab department with several specialties and Eden Prairie is a positive, energetic Rehab Sports & PT department.

This physical therapist will provide outpatient adult pelvic health and orthopedics PT services. Preferred candidate will have experience in providing interventions for adult pelvic health populations. Within pelvic health, the ability to provide female, male, and gender affirming care is ideal. Where training and experience is limited, M Health Fairview offers mentoring and development opportunities for professional growth.

  • FTE: flexible to hire .8 - 1.0 FTE (64-80 hours per pay period).
  • Schedule: Monday - Friday.

M Health Fairview Rehabilitation offers a broad range of services that serve patients across ten acute-care hospitals, three post-acute settings and 70 outpatient adult and pediatric therapy clinics. Consisting of Physical, Occupational and Speech Therapy as well as Audiology and Cardiac & Pulmonary Rehab, our therapists collaborate with colleagues in all medical settings and offer dozens of specialty programs.

As an academic health system with residency and fellowship programs and a rehab-focused clinical quality team, we have a collaborative culture that is centered on learning with an emphasis on evidence based, patient-centered care. Rehab’s continuing education program offers 40+ continuing education courses per year at no cost to employees and further supports numerous external CE courses each year.

As an industry leader in rehabilitative care with nationally recognized programs, our therapists play a critical role in helping patients reach their goals and live more fulfilling lives.

The physical therapist partners with the patient and their families while performing evaluation and re-evaluation.

Develop an individualized plan of care through collaboration with the patient and family.

Capture and apply pertinent data, perform examinations, and determine treatment diagnoses.

Analyzes data to identify needs and potential risks, settles prognosis for recovery, and establishes individualized goals.

Implements and modifies care plan based on reassessment and patient response.

Coordinates care and communicates effectively with interdisciplinary team for all aspects of care & establishes appropriate discharge plan.

Partner with patients and families to provide skilled intervention.

Implements and alters treatment to reflect change in patient status and response to treatment.

Assesses patient and/or caregiver learning needs and provides appropriate education.

Documentation/Billing

Uses acceptable medical terminology and follows professional practice standards.

Professional Expectations

Prioritizes tasks and patient care for effective time management.

Completes documentation in a timely manner per department standards.

Stays in sync with department billing standards.

Meets or exceeds departmental standards for productivity.

Identifies opportunities for process/quality improvement.

Crafting and implementing improvements, and participates in and supports entity/system initiatives.

Understand and maintain licensure/certification requirements.

Advises team members and/or supervises students as designated.

Pursues professional development to support clinical standard processes.

Required License/Certification/Registration

Physical Therapy registration/licensure in the state of Minnesota or temporary license in MN.

Current Basic Life Support (BLS) certification.

Preferred Education & Experience

Doctorate degree in Physical Therapy.

1+ year of Physical Therapy practice.

1+ year experience with pelvic health.
