Job summary

Tridant Solutions is seeking a Physical Therapist to service in the Stockton, California area. The role focuses on evaluating and documenting patient progress, coordinating with IDT teams, and delivering targeted therapy to improve movement and function.

The position emphasizes supervision of therapy staff, hands-on PT interventions, and collaboration with caregivers to ensure quality care. California licensure or eligibility and CPR certification are required.

Qualifications

• Bachelor’s Degree in Physical Therapy with at least 12 months of PT experience.
• Master’s or Doctorate in Physical Therapy is preferred.
• Valid California Physical Therapy License or eligibility required.
• Two years of PT experience with frail or elderly populations preferred.
• Experience supervising PT Assistants and Aides; CPR/BLS certified.

Responsibilities

• Conduct Physical Therapy assessments in facility and home to define care needs and goals.
• Actively participate in IDT meetings to develop participant care plans.
• Deliver and document PT interventions per orders and care plans.
• Provide active/passive exercises, gait training, and ADL functional training.
• Supervise Therapy Assistants and provide training for Functional Maintenance Programs.
• Instruct staff, participants, and caregivers in use/maintenance of equipment.
