Job summary

Homeside Rehab is seeking a compassionate Pediatric Physical Therapist or Occupational Therapist to join our home health team, delivering evidence-based therapy to children in their homes with a flexible schedule and supportive team.

You will evaluate, plan, and implement PT/OT interventions to improve motor skills and developmental outcomes, while ensuring accurate documentation and collaborative care with the clinical team.

Qualifications

• Active RI Physical Therapy License OR Occupational Therapist License — required.
• Basic computer proficiency for electronic documentation.
• Strong clinical assessment and interpersonal communication skills.
• Ability to work independently in a home health environment.
• Valid RI driver’s license and reliable transportation for home visits.
• Pediatric experience is preferred.
• Minimum 1 year of experience in Home Health, Outpatient, or SNF settings.

Responsibilities

• Evaluate patients and develop individualized treatment plans based on clinical assessment and patient goals.
• Provide evidence-based PT/OT interventions to improve motor skills, functional independence, and developmental outcomes.
• Maintain accurate, timely documentation of all patient care in accordance with regulatory requirements.

Job Details:

• Job Type: Full-time, Part-time, Per Diem
• Location: Providence, RI 02909
• Salary: $65 - $80 per hour
• Schedule Flexibility: Choose your own hours, day and evening shifts available, Monday to Friday, with weekend availability as needed
