Job summary

Homeside Rehab is seeking a Pediatric Physical Therapist to join our pediatric home health team in Providence, RI. You will deliver individualized, evidence-based therapy to pediatric patients in their homes, with schedule flexibility and a supportive team environment.

The role offers Full-time, Part-time, and Per Diem options, with a competitive hourly rate and opportunities for bonuses based on performance. A RI license and driving to patient homes are essential for this fast-paced role.

Qualifications

• Active RI Physical Therapy License (PTA) required.
• Pediatric home health experience preferred.
• Minimum 1 year of home health, outpatient, or SNF experience.
• Strong clinical assessment and communication skills.
• Valid RI driver's license and reliable transportation for home visits.

Responsibilities

• Evaluate patients and develop individualized treatment plans based on clinical assessment and patient goals.
• Provide evidence-based PT interventions to improve motor skills, functional independence, and developmental outcomes.
• Maintain accurate, timely documentation of all patient care in accordance with regulatory requirements.
• Collaborate with the clinical team to ensure coordinated and effective treatment.
• Communicate clearly with office staff, patients, and caregivers regarding progress and care plans.

Job Summary: Homeside Rehab is seeking a compassionate and skilled Physical Therapist or PTA to join our pediatric home health team. In this role, you will deliver individualized, evidence-based therapy to pediatric patients in their homes, making a meaningful impact while enjoying schedule flexibility and a supportive team environment.

Job Responsibilities:

• Evaluate patients and develop individualized treatment plans based on clinical assessment and patient goals
• Provide evidence-based PT interventions to improve motor skills, functional independence, and developmental outcomes
• Maintain accurate, timely documentation of all patient care in accordance with regulatory requirements
• Collaborate with the clinical team to ensure coordinated and effective treatment

Job Qualifications:

• Basic computer proficiency for electronic documentation
• Strong clinical assessment and interpersonal communication skills

• Minimum 1 year of experience in Home Health, Outpatient, or Skilled Nursing Facility (SNF) settings
