Job summary

Sweetwater Care seeks a licensed Physical Therapist to evaluate and treat patients, develop individualized plans, and supervise PT assistants. You will review histories, teach therapeutic techniques, and ensure documentation meets regulatory standards.

Responsibilities

• Consult with patients to learn about their physical condition and symptoms; review medical history and referrals.
• Diagnose movement dysfunction by listening to patients and observing them as they move.
• Develop individualized treatment plans and coach patients on therapeutic massage to ease pain and promote healing.
• Maintain patient records and track progress toward goals.
• Utilize standardized assessments and evidence-based practice to support interventions.
• Assess progress and adjust treatments in the clinical record per regulatory guidelines.
• Evaluate treatment outcomes and modify services; refer as appropriate.
• Provide consultation to patients, families, and other service providers regarding physical disorders.
• Communicate with supervisor and health team about patient progress and plans.
• Provide discharge summaries in compliance with regulatory requirements.
• Document all required information in the electronic system and maintain patient records.
• Participate in required trainings, in-services, and conferences.
• Supervise and support PT assistants, rehabilitation aides, and/or students.
• Adhere to company policies and code of conduct.
• Maintain active state licensure.

Principal Responsibilities

• Coach patient and teach them therapeutic massage to ease patients’ pain and provide stimulation to promote healing
• Maintain patient record by keeping track of goals
• Utilize standardized assessments and evidence-based practice to support clinical interventions.
• Assesses patient’s progress and adjust treatments accordingly in the clinical record per regulatory and clinical practice requirements.
• Evaluates treatment outcomes for effectiveness. Services are modified in relation to patient evaluation and referrals are made in relation to outcomes.
• Provides consultation and counseling to patients, as well as families, caregivers and other service providers related to the physical disorders.
• Effectively communicates with supervisor and other health team members regarding patient progress, barriers, and treatment plans.
• Provides comprehensive discharge summary of services provided per regulatory and clinical requirements.
• Documents all regulatory and clinically required information in the electronic documentation system and maintains patient records according to company policy. Accurately records treatment minutes for services provided per all regulatory and clinical requirements.
• Participate in facility and company required trainings, in-services and conferences.
• Able to supervise and support physical therapist assistants; rehabilitation aides and/or students.
• Adherence to Company required policy and procedures and code of conduct.
• Active State Licensure
