Job summary

Sweetwater Care in California seeks qualified clinicians to deliver rehabilitation services, assess conditions, design individualized treatment plans, and coach patients in therapeutic massage to ease pain and promote healing.

You will maintain detailed patient records, collaborate with the health team, and ensure regulatory compliance. Active state licensure and ongoing trainings are required.

Responsibilities

• Consult with patients to learn about their physical condition and symptoms.
• Review patients' medical history and referrals from doctors or surgeons.
• Diagnose movement dysfunction by listening to patients and observing them as they move.
• Develop individualized treatment plan.
• Coach patients and teach therapeutic massage to ease pain and promote healing.
• Maintain patient records by tracking goals.
• Utilize standardized assessments and evidence-based practice to support clinical interventions.
• Assess patient progress and adjust treatments in the clinical record per regulatory requirements.
• Evaluate treatment outcomes and modify services based on evaluations.
• Provide consultation and counseling to patients, families, caregivers and other providers.
• Effectively communicate with supervisor and health team members regarding patient progress, barriers, and treatment plans.
• Provide discharge summaries of services per regulatory requirements.
• Document regulatory and clinical information in the electronic system and maintain patient records; record treatment minutes.
• Participate in trainings, in-services and conferences.
• Supervise and support physical therapist assistants and students.
• Adhere to company policies, procedures and code of conduct.
• Active State Licensure.

• Assesses patient’s progress and adjust treatments accordingly in the clinical record per regulatory and clinical practice requirements.
• Evaluates treatment outcomes for effectiveness. Services are modified in relation to patient evaluation and referrals are made in relation to outcomes.
• Provides consultation and counseling to patients, as well as families, caregivers and other service providers related to the physical disorders.
• Effectively communicates with supervisor and other health team members regarding patient progress, barriers, and treatment plans.
• Provides comprehensive discharge summary of services provided per regulatory and clinical requirements.
• Documents all regulatory and clinically required information in the electronic documentation system and maintains patient records according to company policy. Accurately records treatment minutes for services provided per all regulatory and clinical requirements.
• Participate in facility and company required trainings, in-services and conferences.
• Able to supervise and support physical therapist assistants; rehabilitation aides and/or students.
• Adherence to Company required policy and procedures and code of conduct.
• Active State Licensure
