Job summary

Sweetwater Care in Bakersfield, CA is seeking a Licensed Physical Therapist to assess movement dysfunction, develop individualized treatment plans, and supervise PT assistants on site.

Responsibilities

• Consult with patients to learn about their physical condition and symptoms, review their medical history and referrals.
• Diagnose movement dysfunction by listening to patients and observing them as they move.
• Develop individualized treatment plan.
• Coach patient and teach therapeutic massage to ease patients’ pain and provide stimulation to promote healing.
• Maintain patient records and track goals.
• Utilize standardized assessments and evidence-based practice to support clinical interventions.
• Assess patients' progress and adjust treatments accordingly in the clinical record per regulatory and clinical practice requirements.
• Evaluate treatment outcomes and modify services in relation to patient evaluation and referrals.
• Provide consultation and counseling to patients, families, caregivers, and other service providers related to physical disorders.
• Effectively communicate with supervisor and health team members regarding patient progress, barriers, and treatment plans.
• Provide comprehensive discharge summaries of services per regulatory and clinical requirements.
• Document all regulatory and clinically required information in the electronic documentation system and maintain patient records.
• Participate in facility and company trainings, in-services and conferences.
• Able to supervise and support PT assistants, rehabilitation aides and/or students.
• Adhere to company policies and code of conduct.
• Maintain Active State Licensure
