Job summary

Sweetwater Care in Visalia, CA is seeking a licensed Physical Therapist to evaluate patients, develop individualized treatment plans, and supervise therapy staff. You will document patient progress and outcomes, communicate with the care team, and ensure compliance with regulatory requirements.

You will coach patients on therapeutic movements, assess progress, adjust plans as needed, and participate in trainings. A strong clinical skills background and active licensure are essential.

Responsibilities

• Consult with patients to learn about their physical condition and symptoms; review medical history and referrals.
• Diagnose movement dysfunction by listening to patients and observing them as they move.
• Develop individualized treatment plans.
• Coach patients and teach therapeutic massage to ease pain and promote healing.
• Maintain patient records by tracking goals and progress.
• Utilize standardized assessments and evidence-based practice to support interventions.
• Assess progress and adjust treatments in the clinical record per regulatory requirements.
• Evaluate treatment outcomes and modify services accordingly; refer as needed.
• Provide counseling to patients, families, caregivers, and other providers regarding physical disorders.
• Communicate progress, barriers, and plans with supervisor and health team members.
• Provide discharge summaries per regulatory requirements.
• Document all required information in electronic records and maintain patient files.
• Participate in trainings and in-services.
• Able to supervise physical therapy assistants, aides, and/or students.
• Adhere to company policies and code of conduct.

Principal Responsibilities

• Utilize standardized assessments and evidence-based practice to support clinical interventions.
• Assesses patient’s progress and adjust treatments accordingly in the clinical record per regulatory and clinical practice requirements.
• Evaluates treatment outcomes for effectiveness. Services are modified in relation to patient evaluation and referrals are made in relation to outcomes.
• Provides consultation and counseling to patients, as well as families, caregivers and other service providers related to the physical disorders.
• Effectively communicates with supervisor and other health team members regarding patient progress, barriers, and treatment plans.
• Provides comprehensive discharge summary of services provided per regulatory and clinical requirements.
• Documents all regulatory and clinically required information in the electronic documentation system and maintains patient records according to company policy. Accurately records treatment minutes for services provided per all regulatory and clinical requirements.
• Participate in facility and company required trainings, in-services and conferences.
• Able to supervise and support physical therapist assistants; rehabilitation aides and/or students.
• Adherence to Company required policy and procedures and code of conduct.
