Responsibilities

• Evaluate residents to determine PT needs and rehabilitation potential.
• Develop and update individualized treatment plans to improve mobility and independence.
• Supervise PT Assistants and train staff.
• Document in medical records accurately and timely.
• Educate residents and families on goals, home programs, safety and fall prevention.
• Maintain safe treatment environment and infection control.
• Report equipment issues and contribute to quality initiatives.
