Physical Rehabilitation Network (PRN) and its Family of Brands is hiring a Physical Therapist in Concord, CA to deliver high-quality rehabilitation services. You will assess patients, design individualized treatment plans, and guide them toward improved mobility and pain management.

Qualifications

• Valid state licensure as a Physical Therapist.
• CPR certification and any other required state certifications.
• Strong clinical skills in orthopedic, neurological, or geriatric physical therapy.
• Excellent communication and interpersonal skills.
• Ability to work independently and in a team.
• Proficient documentation/computer skills.

Responsibilities

• Conduct comprehensive assessments to determine functional abilities and goals.
• Develop personalized treatment plans based on assessments.
• Administer therapeutic exercises, manual therapy, and other interventions.
• Maintain accurate documentation of progress and outcomes.
• Educate patients and families on therapy procedures and home exercises.
• Collaborate with physicians, nurses, and other professionals.
• Stay updated on PT techniques and best practices.
