Sayville Rehabilitation & Nursing Center is seeking an experienced Physical Therapist (PT) to join Luxor at Sayville Rehabilitation and Nursing Center. The PT will assess referred patients, design and supervise rehabilitation programs, and work under physician direction.

The ideal candidate will have a PT degree, an active state license, and experience in long‑term or geriatric care. Documentation, patient education, and teamwork with the care team are essential facets of the role.

Qualifications

• Must have graduated from an accredited Physical Therapy college or university.
• Must have a valid professional license and registration for state of practice.
• Collaborates with all disciplines to provide best patient centered treatment and care.
• Experience with long-term care and/or geriatric care is preferred.

Responsibilities

• Assess referred patients requiring rehabilitation and provide direct patient care.
• Establish and administer a treatment program with goals per patient capacity and tolerance.
• Document and record patient condition; educate patients and families in therapy methods.
• Oversee and supervise Physical Therapy Assistants.
• Ensure compliance with confidentiality and regulatory requirements.
