Job summary

Luxor at Sayville Rehabilitation and Nursing Center is seeking an experienced Physical Therapist to join our facility. You will assess referred patients, develop treatment plans, and document progress under physician direction.

You will supervise Physical Therapy Assistants, collaborate with the care team, and educate patients and families on therapy goals. Competitive pay and benefits are offered.

Qualifications

• Must have graduated from an accredited Physical Therapy college or university.
• Must have a valid professional license and registration for state of practice.
• The PT is responsible for supervising Physical Therapy Assistants.
• Experience with long-term care and/or geriatric care is preferred.
• Well versed with documentation writing.
• Evaluations, weekly progress notes, recertification and discharge documentations.
• Collaborates with all disciplines to provide best patient centered treatment.
• Exceptional patient service skills and professionalism.

Responsibilities

• Assess referred patients requiring rehabilitation and provide direct patient care.
• Establish and administer treatment programs with goals based on patient capacity.
• Document and record patient condition and educate patients and families in therapy methods.
• Oversee and supervise Physical Therapy Assistants.
• Collaborate with all disciplines to provide patient-centered care.
