Job summary

Village Park Post Acute in Eugene, OR is seeking a Physical Therapist to restore function, improve mobility, relieve pain, and prevent or limit physical disabilities in residents and patients. You will develop treatment plans, assess needs, collaborate with a multidisciplinary team, set goals, document progress, educate patients and families, and ensure timely, reliable care and communication.

Qualifications

• Bachelor’s, Master’s or Doctorate Degree from an accredited Physical Therapy program.
• Current Physical Therapist license in the state employed.
• Current CPR/BLS certification.
• Six months experience in a long-term care environment preferred.

Responsibilities

• Establishes and administers a treatment plan with goals determined by the physician.
• Assesses patient needs; plans for, evaluates and modifies care to meet therapy goals.
• Collaborates with all disciplines to plan and evaluate team goals for each patient.
• Sets realistic and achievable goals for patients/residents.
• Documents and records the patients’/residents’ condition and progress per regulations.
• Educates patients and families in an appropriate therapy method.
• Optimizes functionality by screening and guiding residents.
• Communicates patient status to patient, family, and other professionals.
• Ensures punctuality and regular attendance for assigned shifts.
