Qualifications

• Bachelor's, Master's or Doctorate in Physical Therapy from an accredited program.
• Current Physical Therapist license in the state of employment.
• Current CPR certification.
• Six months of long-term care experience preferred.

Responsibilities

• Establishes and administers a treatment plan with goals based on the patient’s capacity under physician guidance.
• Assesses patient needs and plans, evaluates, and modifies care to meet therapy goals.
• Collaborates with other disciplines to plan and evaluate patient care goals.
• Sets realistic, achievable goals for patients/residents.
• Documents and records patient status and progress per regulations.
• Educates patients and families on therapy methods.
• Optimizes patient functionality through screening and interventions.
• Communicates patient status to families and involved professionals.
• Maintains punctuality and regular attendance for assigned shifts.
