Flagship Therapy in Modesto, CA is seeking a Physical Therapist Licensed Applicant (PTLA) for our in-house rehab program. This full-time position offers a stable schedule and a chance to work within an expanding network of Ensign-affiliate facilities.

You will conduct screenings, develop care plans, deliver rehabilitative treatment, and prepare discharge summaries while collaborating with the care team. New grads are welcome to apply; licensure is required.

Responsibilities

• Conduct effective screening and evaluation of patients with physical functioning disorders and/or wound care, developing appropriate care plans adhering to regulatory and clinical standards.
• Deliver rehabilitative, skilled, and medically necessary treatment interventions to patients with physical functioning disorders and/or wound care, in accordance with regulatory and clinical practice requirements.
• Administer various procedures as part of the rehabilitation plan, including manual techniques, ambulation, therapeutic exercises, modalities, use of supportive and assistive devices, and wound care.
• Provide consultation and counseling to patients, families, caregivers, and other service providers regarding physical disorders.
• Generate comprehensive discharge summaries of services provided, ensuring compliance with regulatory and clinical requirements.

Join our IN-HOUSE Team in Modesto as a dedicated PTLA!

Our expanding network of in-house therapy Ensign-affiliate operatedhealthcare facilities is growing, and we are scouting for excellent therapists to join us at Vintage FaireNursing and Rehab. We offer a stable and fun in-house therapy team to work with as well as excellent programs serving our geriatric clientele in their journey to recovery. We are currently seeking a Full Time Physical Therapist Licensed Applicant (PLTA) for our in-house rehab program!

Full Time : Flexible schedule with work-life balance

Pay : $55- $62/ hour based on experience

Key Responsibilities:
