Job summary

Life Care Centers of America seeks a Physical Therapist to provide direct rehab services to patients in a skilled nursing setting. The PT will follow physician referrals, implement established care plans, and collaborate with an interdisciplinary team.

Qualifications

• Graduate of an accredited physical therapy program (BSPT/MSPT/DPT).
• Currently registered/licensed in the applicable state; active license required.
• One year of experience preferred; new grads welcome to apply.
• CPR certification required at hire or during orientation.

Responsibilities

• Follow and implement physical therapy treatment plans for patients.
• Establish and adjust goals; assess progress and modify plans.
• Supervise PTAAs and Rehab Aides; oversee care quality.
• Document promptly in therapy software and chart patient progress.
• Collaborate with interdisciplinary team to coordinate care.
• Maintain professional conduct and patient confidentiality.
