Continuum Therapy Partners is seeking a Physical Therapist to contribute to a clinician-driven care model. Based in Winter Park, FL, you’ll join a network that has scaled from nine facilities in NC to 275 facilities across 16 states, always prioritizing the therapist voice and patient outcomes.

The role requires a DPT, state licensure, and NPTE eligibility, with a preference for 1 year of PT experience. A collaborative, patient-centered approach is essential.

Qualifications

• DPT degree required.
• Current, unencumbered state Physical Therapy License; NPTE required.
• 1 year of previous experience as a Physical Therapist preferred.
• Evaluates, develops, and implements individualized treatment plans in coordination with other disciplines.
• Documents patient progress and adjusts plans of care based on clinical outcomes.
• Educates patients and families on home exercise programs and injury prevention.
