Job summary

One Home Health is seeking an Experienced Home Health Physical Therapist (PT) to evaluate and treat patients in their homes across Joliet and surrounding communities. The role offers flexible scheduling with options for Full-Time, Part-Time, or PRN/Per Diem, and compensation ranging from $75.00 to $85.00 per hour, based on experience and credentials.

Qualifications

• Active IL PT license.
• Graduate of an accredited Physical Therapy program.
• Current CPR/BLS certification.
• Valid driver's license and reliable transportation.
• Excellent communication and time-management skills.
• Ability to work independently in home health.

Responsibilities

• Perform comprehensive physical therapy evaluations in patients' homes.
• Develop and implement individualized treatment plans to improve mobility, strength, balance, and functional independence.
• Provide skilled physical therapy interventions according to physician orders and established care plans.
• Assess patients' home environments and recommend safety modifications and assistive devices as appropriate.
• Educate patients and caregivers on exercises, injury prevention, fall prevention, and home safety.
• Collaborate with physicians, nurses, occupational therapists, speech therapists, and other members of the interdisciplinary care team.
• Monitor patient progress and adjust treatment plans based on clinical findings.
• Complete accurate, timely, and compliant clinical documentation.
• Participate in discharge planning and patient outcome evaluations.

Compensation

Job Type:Full-Time, Part-Time, PRN/Per Diem

Qualifications

Required

• ·Active Illinois Physical Therapist (PT) license.

• ·Graduate of an accredited Physical Therapy program.

• ·Current CPR/BLS certification.

• ·Valid driver's license and reliable transportation.

• ·Excellent communication, organization, and time-management skills.

• ·Ability to work independently in a home health setting.

Preferred

• ·Home health experience preferred.

• ·Experience with Medicare home health regulations and documentation requirements.

• ·Knowledge of OASIS documentation.

• ·Experience with Axxess software.

• ·Strong orthopedic, neurological, geriatric, or rehabilitation background.

• ·Bilingual candidates are encouraged to apply.
