Job summary

One Home Health is seeking a Experienced Home Health Physical Therapist (PT) to provide in-home patient care in Lake County and surrounding communities. You will evaluate, plan, and implement personalized PT programs to improve mobility, strength, balance, and independence.

Qualifications

• Active Illinois Physical Therapist (PT) license.
• Graduate of an accredited Physical Therapy program.
• Current CPR/BLS certification.
• Valid driver's license and reliable transportation.
• Excellent communication, organization, and time-management skills.
• Ability to work independently in a home health setting.

Responsibilities

• Perform comprehensive physical therapy evaluations in patients' homes.
• Develop and implement individualized treatment plans to improve mobility, strength, balance, and functional independence.
• Provide skilled physical therapy interventions according to physician orders and established care plans.
• Assess patients' home environments and recommend safety modifications and assistive devices as appropriate.
• Educate patients and caregivers on exercises, injury prevention, fall prevention, and home safety.
• Collaborate with physicians, nurses, occupational therapists, speech therapists, and other members of the interdisciplinary care team.
• Monitor patient progress and adjust treatment plans based on clinical findings.
• Complete accurate, timely, and compliant clinical documentation.
• Participate in discharge planning and patient outcome evaluations.

Experienced Home Health Physical Therapist (PT)

One Home Health serves patients throughout Lake County and surrounding communities. Multiple service territories are available, and assignments are made based on patient needs, clinician preference, and geographic location whenever possible.

Compensation

Job Type:Full-Time, Part-Time, PRN/Per Diem
Pay: $75.00 - $85.00 per hour Compensation is based on experience, qualifications, and clinical expertise.
Location: Lake County and surrounding communities

• Experience with Medicare home health regulations and documentation requirements.

• Knowledge of OASIS documentation.

• Experience with Axxess software.

• Strong orthopedic, neurological, geriatric, or rehabilitation background.
