Job summary

One Home Health is seeking a dedicated Experienced Home Health Physical Therapist (PT) to provide in-home assessments and treatments across Valparaiso and surrounding Northwest Indiana communities.

You will perform evaluations, develop personalized plans, and collaborate with a multidisciplinary team to improve mobility and independence in patients’ homes. Excellent communication and independence are essential, with flexible scheduling and strong leadership support from the clinic.

Qualifications

• Active Illinois Physical Therapist (PT) license.
• Graduate of an accredited Physical Therapy program.
• Current CPR/BLS certification.
• Valid driver’s license and reliable transportation.

Responsibilities

• Perform comprehensive physical therapy evaluations in patients' homes.
• Develop and implement individualized treatment plans to improve mobility, strength, balance, and functional independence.
• Provide skilled physical therapy interventions according to physician orders and established care plans.
• Assess patients' home environments and recommend safety modifications and assistive devices as appropriate.
• Educate patients and caregivers on exercises, injury prevention, fall prevention, and home safety.
• Collaborate with physicians, nurses, occupational therapists, speech therapists, and other members of the interdisciplinary care team.
• Monitor patient progress and adjust treatment plans based on clinical findings.
• Complete accurate, timely, and compliant clinical documentation.
• Participate in discharge planning and patient outcome evaluations.

One Home Health serves patients throughout Valparaiso and surrounding communities. Multiple service territories are available, and assignments are made based on patient needs, clinician preference, and geographic location whenever possible.

Compensation

Job Type:Full-Time, Part-Time, PRN/Per Diem
Pay: $65.00 - $75.00 per hour Compensation is based on experience, qualifications, and clinical expertise
Location: Valparaiso and Surrounding Northwest Indiana Communities

• Experience with Medicare home health regulations and documentation requirements.
• Knowledge of OASIS documentation.
• Experience with Axxess software.
• Strong orthopedic, neurological, geriatric, or rehabilitation background.
• Bilingual candidates are encouraged to apply.
