Job Summary and Qualifications

The Physical Therapist provides patient-centered care, managing therapeutic processes for assigned patients, coordinating with physicians and care teams. Responsibilities include assessments, developing and implementing care plans, and supporting patients throughout their care continuum.

What You Will Do

• Conduct evaluations and treatment interventions within legal and ethical scope.
• Assess and evaluate therapeutic and functional status, participate in care planning.
• Provide therapeutic care at patients' homes, using appropriate procedures and techniques.
• Educate patients, families, and caregivers on therapies, exercises, and devices.
• Prioritize caseload, manage staffing needs, and coordinate assignments.

Qualifications Needed

• Basic Cardiac Life Support certification within 30 days of employment.
• Licensed Physical Therapist.
• Bachelor's Degree.
