Job Summary and QualificationsThe Physical Therapist (PT) delivers patient-centered care, managing therapeutic processes for assigned patients in coordination with physicians and the care team. Responsibilities include patient assessments, developing individualized care plans, and providing or supervising physical therapy treatments across the continuum of care.
What You Will Do- Perform evaluations and interventions within legal and ethical practice standards.
- Assess and develop care plans based on functional status and therapeutic needs.
- Deliver therapeutic care at patients' homes, using appropriate procedures and techniques.
- Educate patients and caregivers on therapies, exercises, and devices outside of sessions.
- Manage caseloads, staffing, and patient handoffs effectively.
Qualifications Needed- Basic Cardiac Life Support certification within 30 days of employment.
- Licensed Physical Therapist.
- Bachelor's Degree.
