Job Summary and Qualifications

The Physical Therapist (PT) provides high-quality, patient-centered care, managing the therapeutic process for assigned patients in coordination with physicians and the care team. Responsibilities include patient assessments, developing individualized care plans, and delivering or directing therapy to support patients across the continuum of care.

What You Will Do In This Role

• Provide evaluations and treatment interventions within legal and ethical practice.
• Assess and evaluate therapeutic/rehabilitative/functional status and contribute to care planning.
• Deliver purposeful and skilled therapy at the patient's location, using appropriate procedures and techniques.
• Instruct patients, families, and caregivers on therapies, exercises, and device use outside of sessions.
• Prioritize caseload, determine staffing needs, and coordinate assignments.

What Qualifications You Will Need

• Basic Cardiac Life Support certification within 30 days of start date.
• Licensed Physical Therapist.
• Bachelor's Degree.
