Job Summary and Qualifications

The Physical Therapist (PT) provides high-quality, patient-centered care by managing therapeutic care for assigned patients, coordinating with physicians and the care team. Responsibilities include patient assessments, developing and implementing care plans focused on goals, engagement, and positive experiences. The PT delivers or oversees most physical therapy treatments supporting patients throughout their care continuum.

What You Will Do in This Role

• Provide evaluations and treatment interventions within legal and ethical practice standards.
• Assess therapeutic/rehabilitative/functional status and participate in care planning.
• Administer purposeful and skilled therapy at patients' homes, using appropriate procedures and techniques.
• Instruct patients, families, and caregivers on therapies, exercises, and devices to be used outside sessions.
• Prioritize caseload, determine staffing needs, and coordinate assignments.

What Qualifications You Will Need

• Basic Cardiac Life Support (BLS) certification obtained within 30 days of employment.
• Licensed Physical Therapist.
• Bachelor's Degree in relevant field.
