Evolve Therapy Services in Minnesota seeks a dedicated Physical Therapist for an in-house Skilled Nursing Facility in Franklin to join our therapy team and deliver high-quality resident care.

You will assess residents, develop and implement treatment plans, document progress per CMS and NetHealth, and collaborate with an interdisciplinary team. A BS in Physical Therapy and an active license are required, with ongoing CE opportunities.

Qualifications

• Bachelor of Science in Physical Therapy from an accredited program.
• Active Physical Therapy license in practicing states.
• Ongoing continuing education to maintain licensure and certification.

Responsabilités

• Provide physical therapy services to residents per standards and procedures.
• Document and bill PT services per procedures.
• Develop and implement treatment plans with short- and long-term goals.
• Conduct screenings and evaluations to determine need for intervention/treatment.
• Communicate evaluation findings and recommendations to residents and interdisciplinary team.
• Complete CMS-compliant documentation in NetHealth.
