Qualifications

• Bachelor of Science in Physical Therapy from an accredited program.
• Active Physical Therapy license in state(s) of practice.
• Ongoing continuing education to maintain licensure and certification.
• Maintain current licensure and certification.

Responsibilities

• Provide physical therapy services to residents according to standards and procedures.
• Document and bill services per CMS expectations.
• Evaluate residents to plan and implement therapy.
• Develop and adjust treatment plans with measurable goals.
• Communicate findings and plans to residents and the care team.
• Complete required documentation in NetHealth EMR systems.
