Job summary

Evolve Therapy Services in Richmond, VA, is hiring compassionate Physical Therapists for an in-house Skilled Nursing Facility partner. You will provide patient-centered PT services and collaborate with an interdisciplinary team to enhance resident mobility and quality of life.

Responsibilities include evaluations, treatment planning, progress documentation in NetHealth EMR, and compliance with CMS guidelines.

Qualifications

• Bachelor of Science in Physical Therapy from an accredited program.
• Must hold an active Physical Therapy license in state(s) of practice.
• Ongoing continuing education to maintain licensure.

Responsibilities

• Provide physical therapy services per standards and productivity guidelines.
• Document and bill therapy services accurately.
• Evaluate residents and develop treatment plans with measurable goals.
• Screen residents regularly to determine intervention needs.
• Communicate findings and plans to residents, families, and interdisciplinary team.
• Maintain compliant CMS documentation in NetHealth EMR.
