Job summary

Evolve Therapy Services, LLC is seeking a Physical Therapist to provide in-house SNF PT services at our Richmond, VA partner facility. You will plan, organize, and direct PT services in accordance with federal, state and local standards while maintaining high resident care quality.

Responsibilities include evaluating residents, developing treatment plans with goals, documenting CMS-compliant care, and collaborating with an interdisciplinary team.

Qualifications

• BS in Physical Therapy from an accredited program.
• Active Physical Therapy license in state(s) of practice.
• Ongoing continuing education to maintain licensure/certification.

Responsibilities

• Provide physical therapy services per standards of practice and department procedures.
• Document and bill PT services in accordance with company procedures.
• Evaluate residents and develop treatment plans with short and long-term goals.
• Screen residents at regular intervals to determine intervention needs.
• Screen residents referred for PT to determine intervention need.
• Interpret evaluation findings and communicate plans to residents and families.
• Discharge responsibilities and ensure CMS compliance in documentation.
• Complete compliant documentation in NetHealth EMR systems.
• Collaborate with interdisciplinary team for resident care.
• Perform duties necessary to discharge responsibilities in best interests of the organization.

Job Position

Physical Therapist for an In-house Skilled Nursing Facility

Job Type

Full Time, Part Time, or PRN positions available

Job Purpose

To assist in planning, organizing, developing, and directing Physical Therapy services in accordance with current applicable federal, state, and local standards, guidelines, and regulations, and as may be direction by Administrator, to assure the highest degree of quality resident care is always maintained.

Job Functions

• Provide physical therapy services to residents in accordance with established standards of practice, Company procedures, Therapy Department procedures and productivity standards.
• Adhere to Company procedures regarding documentation and billing of physical therapy services.
• Demonstrate sound judgment in the evaluation, planning, implementation, and follow-up of resident therapy programs.
• Conduct screening of residents at regular intervals to determine need for intervention/treatment.
• Conduct timely screening of residents referred to physical therapy to determine need for intervention/ treatment.
• Evaluate residents to obtain data necessary for treatment planning and implementation.
• Conduct specialized evaluations as indicated.
• Develop treatment plans by establishing short and long-term goals and methods to achieve identified goals.
• Interpret and communicate evaluation findings, treatment plan and recommendations to residents, families/ responsible parties, and interdisciplinary team members.
• Perform all other duties as assigned and as required to effectively discharge the responsibilities of the position and are in the best interests of the organization.
• To complete compliant and medically necessary documentation from CMS expectations in NetHealth EMR systems

Education & Requirements

• Minimum of Bachelor of Science Degree in Physical Therapy from an accredited program
• Must have an active Physical Therapy license in state(s) of practice
• Ongoing continuing education and professional development to maintain current licensure and certification
• Assume responsibility for ongoing continuing education and professional development to maintain current licensure and certification.
