Job summary

Evolve Therapy Services in Antioch, TN is seeking a compassionate Physical Therapist for an in-house Skilled Nursing Facility. Full-time, part-time, or PRN positions available.

Responsibilities include providing physical therapy services, documenting per standards, evaluating residents, and developing treatment plans with goals. Must hold an active PT license and perform CMS/NetHealth documentation. Growth opportunities are available.

Qualifications

• Bachelor of Science in Physical Therapy from an accredited program.
• Active Physical Therapy license in state(s) of practice.
• Ongoing continuing education to maintain licensure.

Responsibilities

• Provide physical therapy services to residents per standards and procedures.
• Adhere to documentation and billing procedures for therapy services.
• Develop treatment plans with short and long-term goals.
• Conduct resident screenings to determine needs and intervention.
• Interpret evaluation findings and communicate recommendations to residents and team.
• Complete CMS-compliant documentation in NetHealth EMR system.
