Evolve Therapy Services is seeking a compassionate Physical Therapist for an in-house Skilled Nursing Facility in Antioch, TN. This role offers full-time, part-time, or PRN positions and focuses on planning, organizing, and directing PT services to ensure high-quality resident care.

The ideal candidate holds a BS in Physical Therapy, maintains an active state license, and commits to ongoing professional development and accurate CMS-compliant documentation.

Qualifications

• Bachelor of Science in Physical Therapy required.
• Active PT license in state(s) of practice required.
• Ongoing continuing education to maintain licensure.
• CMS/NetHealth documentation expectations to be met.

Responsibilities

• Provide physical therapy services to residents following established standards and procedures.
• Adhere to documentation and billing procedures for therapy services.
• Evaluate and develop treatment plans with short- and long-term goals.
• Conduct resident screenings to determine intervention needs.
• Communicate findings and recommendations to residents, families, and the care team.
• Complete compliant, medically necessary documentation per CMS expectations.
