Qualifications

• Active Physical Therapy license in state(s) of practice.

Responsibilities

• Provide physical therapy services per standards and procedures.
• Document and bill PT services according to CMS expectations.
• Evaluate and plan resident therapy programs and goals.
• Screen residents regularly to identify intervention needs.
• Develop and implement treatment plans with clear goals.
