Job Opportunity: Physical Therapist (PT) at BAYADA Home Health Care

BAYADA Home Health Care is seeking an experienced Physical Therapist (PT) for a full-time role providing home health visits throughout Cuyahoga County. This position serves adult and geriatric clients on a per-visit basis.

Requirements include a minimum of one year of clinical experience as a licensed Physical Therapist. As a home care PT, you will be a vital part of a multidisciplinary team delivering rehabilitative care and skilled nursing, enabling clients to remain at home.

Responsibilities:

• Make home visits to clients in designated areas.
• Perform diagnostic tests such as mobility, strength, balance, and safety assessments.
• Implement wound care interventions using evidence-based dressings and topical agents.
• Develop and revise individualized care plans, including manual therapy, gait training, and balance exercises.
• Assess and update care plans regularly, collaborating with nursing staff.
• Educate clients and families on rehabilitative care to promote health and independence.
• Document all observations and interventions accurately using electronic tablets.

Qualifications:

• Ohio PT license.
• At least one year of recent PT experience.
• Graduation from an accredited physical therapy program.
• Ability to work independently and manage time effectively.
• Strong interpersonal and organizational skills.
• Proficiency with computers; EMR experience preferred.
