Job description

Speech Language Pathologist, Home Health Visits in Senior Living

Join our growing team at BAYADA Home Health Care as an experienced Speech Language Pathologist (SLP) for a per diem opportunity. You will perform home health visits across communities in Uniontown and surrounding areas.

Prior home care experience is a plus but not required. As an SLP, you will be a vital part of a multidisciplinary team providing rehabilitative care and skilled nursing, enabling clients to stay at home.

Responsibilities:

1. Make home visits to clients in designated areas.
2. Conduct evaluations to diagnose and treat speech, language, communication, or swallowing issues.
3. Develop and update treatment plans based on evaluations and physician’s orders.
4. Collaborate with the care team and document care using electronic records.
5. Educate patients and families on rehabilitative activities.

Qualifications:

• Current Ohio license as a Speech Language Pathologist.
• Certificate of clinical competence from ASHA or equivalent education and experience.
• Strong computer skills; EMR experience preferred.
