BAYADA Home Health Care - Physical Therapist (PT) Job Description

Position: Full-Time Physical Therapist (PT) for home health visits in Cuyahoga County's senior living offices.

Responsibilities:

1. Make home visits to clients in designated geographic areas.
2. Perform diagnostic tests and measurements, including mobility, strength, balance, and safety assessments.
3. Implement wound care interventions using evidence-based dressings and topical agents.
4. Develop and execute individualized care plans with manual therapies, gait training, and balance exercises.
5. Assess and revise care plans regularly, collaborating with nursing staff.
6. Educate clients and families on rehabilitative activities to promote health and independence.
7. Document client observations and interventions accurately using electronic records.

Qualifications:

• Current Ohio Physical Therapist (PT) license.
• At least one year of recent clinical experience as a PT.
• Graduation from an accredited Physical Therapy program.
• Ability to work independently and manage time effectively.
• Strong interpersonal, organizational, and problem-solving skills.
• Proficiency with computers; EMR experience preferred.
