Job description

BAYADA Home Health Care is currently seeking an experienced Physical Therapist (PT) for a full-time opportunity performing home health visits for our senior living office, located in Seabrook, Hampton, and Exeter NH. This office services adult and geriatric clients on a per-visit basis across the Seabrook, Hampton, and Exeter area.

One year of prior clinical experience as a licensed Physical Therapist is required. As a home care Physical Therapist, you will be an integral member of a multidisciplinary healthcare team that provides rehabilitative care and skilled nursing to clients, helping them receive the medical care they need to remain at home.

Responsibilities include:

1. Make home visits to clients in designated geographic territories.
2. Perform diagnostic tests and measurements, such as mobility, joint range, transfer status, stability, ambulation patterns, muscle strength and endurance, balance testing, and safety assessments.
3. Develop and implement individualized care plans, including manual therapeutic exercises, gait training, balance, and other interventions.
4. Continuously assess and revise the care plan, collaborating with nursing staff as appropriate.
5. Educate clients and their families on rehabilitative care and activities to promote health, safety, and independence.
6. Document observations, interventions, and evaluations accurately using a state-of-the-art touch pad tablet.

Qualifications:

• Current New Hampshire Physical Therapist - PT license.
• At least one year of recent work experience as a Physical Therapist.
• Graduation from an APTA-approved program or equivalent.
• Ability to work independently and manage time effectively.
• Strong interpersonal, organizational, and problem-solving skills.
• Proficiency with computers; prior EMR experience preferred.
