BAYADA Home Health Care - Physical Therapist (PT) Job Description

Position: Full-time Physical Therapist (PT)

Location: Kennebunk, Maine office

Overview: BAYADA is seeking an experienced Physical Therapist to perform home health visits for adult and geriatric clients throughout Maine. This role involves working as part of a multidisciplinary team to provide rehabilitative care that enables clients to remain at home.

Responsibilities:

1. Make home visits to clients in designated territories.
2. Perform diagnostic tests and measurements related to mobility, strength, balance, and safety.
3. Implement wound care interventions using evidence-based dressings and topical agents.
4. Develop and revise individualized care plans, including manual therapy, gait training, and balance interventions.
5. Assess and update the care plan regularly, collaborating with nursing staff as needed.
6. Educate clients and family members on rehabilitative activities to promote health and independence.
7. Document all observations and interventions accurately using electronic medical records.

Qualifications:

• Current Maine Physical Therapist (PT) license.
• At least one year of recent clinical experience as a PT.
• Graduation from an APTA-approved program or equivalent.
• Ability to work independently and manage time effectively.
• Strong interpersonal, organizational, and problem-solving skills.
• Proficiency with computers; EMR experience preferred.
