WesleyLife Hospice is seeking a licensed Physical Therapist to provide in-home, one-on-one care across Iowa. You will assess mobility, design personalized treatment plans, and deliver targeted therapy to restore function and independence.
You will educate patients and caregivers, coordinate with the care team, and adjust plans as progress dictates, ensuring compassionate, evidence-based support in the patient’s home.
As a Physical Therapist, you’ll make a direct and meaningful impact on the health, comfort, and daily experience of those we serve. You’ll work one‑on‑one with clients in their homes, helping them regain mobility, improve strength, and maintain independence in the place that matters most.
Day in the Life of a Physical Therapist with Wesleylife At Home:
Assess clients in their homes and develop individualized treatment plans that support mobility, safety, and functional goals.
Deliver one‑on‑one therapy, using tailored exercises, manual interventions, gait training, and evidence‑based techniques.
Monitor progress and adjust plans as needs evolve, ensuring clients receive high‑quality, responsive support.
Assess patients at homeand create personalized treatment plans to improve mobility and function.

Educate patients and caregiverson safe practices, pain management, and home modifications.
Collaborate with healthcare teamsto ensure coordinated and continuous care.
