You will deliver PT services per state regulations, document progress, and collaborate with the health care team. This full-time position offers a $10,000 bonus and relocation assistance.

Qualifications

• Current Physical Therapy licensure in the state of practice.
• Valid driver's license and vehicle insurance or access to reliable transportation.
• Current CPR certification or ability to obtain within 90 days of hire.

Responsibilities

• Evaluates patient functional status and PT needs.
• Develops and implements plan of care.
• Provides PT services per state scope and plan of care.
• Observes and reports patient response to treatment and changes in condition.
• Instructs patient and family regarding plan of care and progress.
• Visits patients per treatment plan and documents progress notes.
• Completes patient evaluations and PT plan of care per state guidelines.
• Reports outcomes, goals, and projected frequency of care.
• Adapt care regardless of patient background.
