Job summary

UnitedHealth Group, serving as part of DFW Home Health, seeks a Physical Therapist to assess patient needs, determine treatment plans, and deliver interventions in home health settings. You will evaluate progress, adjust plans, and collaborate with the care team to improve mobility and comfort.

Must hold current PT licensure in the practicing state and have a valid driver's license; CPR is preferred. The role offers a generous total cash target pay with a $10,000 bonus and relocation assistance

Qualifications

• Current Physical Therapy licensure in state of practice.
• Current driver's license and vehicle insurance, access to a dependable vehicle, or public transportation.
• Preferred CPR certification or ability to complete within 90 days of hire.

Responsibilities

• Evaluates the patient's functional status and PT needs.
• Provides services within the scope of practice as defined by state laws and the plan of care.
• Observes and reports the patient's response to treatment and any changes to the supervising nurse or physician.
• Treats patients to relieve pain, develop and restore function, and maintain maximum functioning.
• Instructs and informs patient, family, and care team about the plan of care and progress.
• Visits patients according to Plan of Treatment and submits progress notes per policy.
• Completes evaluations and develops the PT plan of care within state guidelines.
• Reports outcomes and anticipated frequency of care.
