Job Opportunity: Weekend Baylor Physical Therapist (PT) at BAYADA Home Health Care

BAYADA Home Health Care is seeking a compassionate and dedicated Physical Therapist (PT) to join our Forsyth Visits team as a full-time weekend Baylor (Friday, Saturday, Sunday, Monday). This role involves providing services to our adult and geriatric clients across Forsyth, Wilkes, Yadkin, Stokes, and Surry counties, primarily conducting start of care (SOC) visits.

Role Overview

As a Weekend Baylor Physical Therapist, you will be a vital part of a multidisciplinary team delivering rehabilitative care and skilled nursing, enabling clients to remain safely at home.

Responsibilities

• Make home visits within designated territories.
• Perform diagnostic tests and measurements, including mobility, joint range, transfer status, stability, ambulation patterns, muscle strength and endurance, balance, and safety assessments.
• Implement wound care interventions using evidence-based dressings and topical agents.
• Develop and revise individualized care plans, including manual therapy, gait training, balance, and other interventions.
• Assess and update care plans regularly, collaborating with nursing staff as needed.
• Educate clients and their families on rehabilitative activities to promote health and independence.
• Document observations, interventions, and evaluations accurately using electronic tablets.

Qualifications

• Current NC Physical Therapist (PT) license
• Minimum one year of recent PT experience
• Graduation from an American Physical Therapy Association-approved program
• Ability to work independently and manage time effectively
• Strong interpersonal, organizational, and problem-solving skills
• Proficiency with computers; prior EMR experience preferred
