Qualifications

• Current Physical Therapy licensure in state of practice.
• Current driver’s license and vehicle insurance, access to a dependable vehicle, or public transportation.
• Preferred: CPR certification (or ability to obtain within 90 days)

Responsibilities

• Evaluates patient functional status and PT needs; assists in development of the plan of care.
• Provides PT services within scope of practice and plan of care.
• Observes and reports patient response to treatment to supervisor or physician.
• Treats patients to relieve pain and restore or maintain function.
• Instructs patient and family about plan of care, limitations, and progress toward goals.
• Visits patients per Plan of Treatment; completes progress notes per policy.
