Job description

BAYADA Home Health Care is currently seeking an experienced Physical Therapist (PT) for a full-time opportunity performing home health visits for our senior living office. This office services adult and geriatric clients on a per-visit basis in territories throughout the Yorktown, Newport News, and Hampton, VA area.

One year of prior clinical experience as a licensed Physical Therapist is required. As a home care Physical Therapist, you will be an integral member of a multidisciplinary healthcare team that provides rehabilitative care and skilled nursing to clients, enabling them to receive necessary medical care while remaining at home.

Responsibilities for a PT - Physical Therapist include:

1. Make home visits to clients in designated geographic territories.
2. Perform diagnostic tests and measurements, such as mobility, range of joints, transfer status, stability, ambulation patterns, muscle strength and endurance, balance testing, and safety assessments.
3. Implement integumentary protective interventions to prevent wound formation and promote wound repair using evidence-based dressings and topical agents.
4. Develop and implement individualized care plans, including manual therapeutic exercises, gait training, balance, and other interventions.
5. Continuously assess and revise the care plan, collaborating with nursing in the multidisciplinary team as appropriate.
6. Educate and instruct clients, family members, or representatives in rehabilitative care and activities to promote health, safety, and independence.
7. Document observations, interventions, and evaluations accurately using a state-of-the-art touchpad tablet.

Qualifications for a PT - Physical Therapist include:

• A current VA Physical Therapist - PT license.
• At least one year of recent work experience as a Physical Therapist.
• Graduation from an American Physical Therapy Association-approved program or the Committee on Allied Health Education and Accreditation of the American Medical Association, as evidenced by transcript or diploma.
• Ability to work independently and manage time effectively.
• Strong interpersonal, organizational, and problem-solving skills.
• Solid computer skills; prior experience with electronic medical records (EMR) preferred.
