Job Opportunity: Physical Therapist (PT) at BAYADA Home Health Care

BAYADA Home Health Care is seeking an experienced Physical Therapist (PT) for a full-time position providing home health visits within our senior living office. The office services adult and geriatric clients across territories in Yorktown, Newport News, and Hampton, VA.

Applicants must have at least one year of clinical experience as a licensed Physical Therapist. As a home care PT, you will be a vital part of a multidisciplinary team delivering rehabilitative care and skilled nursing, enabling clients to stay safely at home.

Responsibilities:

1. Make home visits to clients in designated areas.
2. Perform diagnostic tests including mobility, strength, balance, and safety assessments.
3. Implement wound care interventions using evidence-based dressings and topical agents.
4. Develop and revise individualized care plans involving manual therapy, gait training, and balance exercises.
5. Collaborate with nursing staff on multidisciplinary care plans.
6. Educate clients and families on rehabilitative activities to promote health and independence.
7. Document care observations, interventions, and evaluations accurately using electronic tablets.

Qualifications:

1. Current VA Physical Therapist license.
2. Minimum of one year recent experience as a PT.
3. Graduation from an American Physical Therapy Association approved program.
4. Ability to work independently and manage time effectively.
5. Strong interpersonal, organizational, and problem-solving skills.
6. Computer proficiency; experience with EMR preferred.
